Description
The Collier County Public Schools Head Start Program will be registering children for the upcoming school year 2012-2013. Parents need to call starting February 1st 2012 and throughout the rest of the year.
REQUIREMENTS FOR QUALIFYING:
*Child must be 4 years old by September 1st of the current year
*Family must meet low income guidelines
AFTER QUALIFYING, FAMILY MUST PROVIDE THE FOLLOWING AT REGISTRATION:
*Proof of Income (Tax Return, W-2's)
*Child's Certified Birth Certificate
*Current Immunization Certificate with Varicella
*Current Physical Exam with Hemoglobin Results
*Parent Photo ID
*Medicaid/Insurance Card
*3 Proofs of Address (Lease, Electricity Bill, 2 Water Bills)
SERVICES FOR CHILDREN WITH DISABILITIES
Transportation must be provided by parents. Neediest families may apply for limited transportation spots
